I know This Is an old post but did you ever find the solution?I am having same issue

I Have a 07 phazer RTX. When I am driving 20 30 40 mph the green drive light goes off and the sled hesitates for a split second as it jumps out of drive, then it goes back in and continues to drive. It is doing this every 20 seconds or so. There is no grinding or anything to make me believe it...
