Yes you do. I use a 14mm crows foot on a long extension to reach in there.
Also it helps if you remove the plastic push pins that hold the bellows boot to the belly pan. Then you can pull the bellows boot back and snake the crows foot and extension in there. ;)!
 
technically you don't have to loosen all four. To make it simpler, pop the outer tie off the spindle and adjust that.

That would work but not very precise since you have to go full rotations at a time.

Actually just a half. The bolt holding the tie on comes out, its not a stud. A half turn is about an 1/8" in toe.
