Jackshaft bearing failure

I have had drive bearing failures twice. This is first time behind secondary. Has anyone experienced this? This bearing was a major pain to get off due to the slip tapered sleve that the bearing rides on. Had to remove jackshaft to cut the bearing off.
How many shims go behind secondary?
 
The amount and size of shims vary. When you set up the clutch there are specs for the amount of offset from the primary and the amount of freeplay in the secondary.

Do you clean and regrease the clutch side bearings every season?
 
Both the jack and track drive shaft bearings, use marine grease which is lithium base and even if it does get some water in it won't rust.
