Any woodworkers or cabinet makers here?

I need a 9" x 12" x 1/2" (approx dimensions) piece of finished soft maple to cover up an access hole that I made.
I would mail you a piece of the molding I took off so you could copy the finish (satin poly?).

I'll provide a drawing of the piece and where the 4 ea. mount holes need to be. The mounting holes will need to be spot faced as I'm going to put in 4 of those decorative "thingy's" to cover the holes.

Can anybody help out there?
