My 06 apex has about 6500 miles on it and last weekend I seen that I had no oil in the chaincase. When I futher inspected there was about a 1 inch hole through chain case housing with a bolt head showing. When I remove chaincase cover it was the bolt near the bottom that goes through a spring then holds on two big gears and threads into the driveshaft. The bolt came completly out and had drilled its way through cover. I think the only thing holding the gears on was the spring pushing against housing. Has anyone had this happen? When I put it back togeather I lock-tited the bolt. Is something else going on or did the bolt just loosen up?

Did you make sure there was no oil in the threads before install?

Looks to me like your reverse is not adjusted properly as the shift dogs are half worm off.

So it has happened to others that looks just like mine. It could have something to do with the reverse adj. how do you know if it is adj right? Mine works fine but I had to spread linkage to make it work

I have seen the bolt backing out on the nytro's but never heard of any Apex's, my 2011 Apex has a c-clip to prevent the bolt from backing out.
Just clean up the threads and locktite and you should be good.

The 06s had issues and there was a updated chaincase cover. Never heard of this though. The issue was the original cover would heat up when running hard and suck the gasket inside the cover which then let all the oil drain out destroying your chaincase most of the time. Had this happen to my 06 RTX. Was fixed under warranty with updated cover and never had another problem. Not sure if this applies to your situation but could I guess.

this can happen with any sled it is just a matter of useing some lock tite on clean threads and remember you may want to get it apart again some day so use the blue stuff if you can. I have had this happen on other sleds and have heard of it on apexes of this era. not uncommon. have the case welded and call it a day.
