Head shims/notched cam timing and idling problems

Does anyone else have idling issues with the notched cam gear and head shim. The idle screw doesn't seem to do anything and I have ran A/F ratios from 10 to 15 and it still doesn't want to idle. When setting the timing the intake cam lines up perfectly with the crank and the exhaust is slightly ahead (towards gas tank) of the mark. Any ideas.

BD turbo, 2 Yamaha gaskets, 1 aluminum shim, notched intake cam gear

The only other I guy I heard of with idling issues ended up luching a couple of pistons because he used the wrong marks for the timing .
 
Might want to recheck the timing, maybe you used the wrong mark on the crank? I line up the marks before I take it apart, then it makes it a lot easier going back together as everything is already really close and fresh in your mind.
