Any where close to the motor , plus dont have it next to a rad , because a rad will cool down the coolent , and wont give you a good estamate of engine temp.

The Phazer is best put in the hose going to the bottom of the radiator. This will make your gauge accurate with the hot light in the OEM gauge.

Does the coolant flow from the engine to the rad then to the heat exchanger or the other way around?

I don't remember which way the coolant flows for sure, but I believe it's to the exchanger first, then the radiator.

If you look at the tank (facing the tank) it will flow towards the front of the sled from the tank. So it will be flowing to the right, in from the rear hole out from the front hole.
